Falls Prevention
First Community Health and Care
First Community’s Falls Prevention Service provides specialist care for patients who are at risk of falling or who have had a fall. They will provide support and care to maximise your independence and minimise your risk of falling in the future.
The team will visit you in your home to assess your mobility, balance and muscle strength, and consider all the main risk factors associated with falls. They will also carry out a home hazards assessment.
Treatment will be planned which could include balance and strength classes, home exercise programme, and equipment and support to help you stay safe and independent.
Referrals can be made by a qualified health care professional or by the residential home manager after completing the pre-referral checklist.
