Social Prescribing in Surrey Heath

Social prescribing is a free service for Surrey Heath residents. It puts you in touch with a wide range of local services and activities in the community to support you to improve your overall wellbeing.

Social prescribing aims to support individuals to take better control over their health and improve their wellbeing. It's designed for people with a range of social, emotional or practical needs that need a little help navigating their way through community-based services and organisations available locally.

Surrey Heath offer one to one appointments with their Wellbeing Adviser where you can talk about your concerns and the factors that affect your health. The Wellbeing Adviser will then help you to identify and organise assistance through Council, community and voluntary organisations available locally.

The Wellbeing Adviser can support you to access the following:

  • support with living independently at home
  • transport to help with getting out and about
  • groups and activities to support with physical and mental health
  • leisure and social activities
  • advice services including benefits, finance and housing 
  • support around a disability 
  • family and carer support services
