Community Stroke and Neurological Rehabilitation and Early Supported Discharge

First Community’s Community Stroke and Neurological Rehabilitation and Early Supported Discharge Service provides specialist assessment and advice for patients and carers dealing with neurological conditions such as stroke, multiple sclerosis, and motor neurone disease.

The teams can support you with:

  • mobility
  • communication
  • community access
  • return to work 
  • eating and drinking
  • washing and dressing
  • managing household tasks
  • memory and thinking skills
  • coming to terms with disability

You will be seen in your own home or at Oxted Therapies Unit. Therapists arrange appointments with you and may set up programmes that can be carried out by Rehabilitation Assistants. The service aims to assess you and agree goals around daily living skills, physical recovery, communication, and memory and concentration in order to maximise your independence in the community.

To be referred patients must be aged over 18, registered with a local GP, and require a short-term goal-directed programme, for a problem related to a neurological condition. Referrals can be received from GPs, local hospitals, or secondary care inpatient rehabilitation.
